Please complete this form to book a place on our ‘UV SPORTS CLUB’ or ‘KS2 Fitness Club’ which will be held during Spring Term 1. The club will run from Wed 8th January until Fri 21st February 2025, from 3.15pm until 4.10pm, for 7 weeks, as follows:.

* Wednesday – Year 1, Year 2 &  Year 3
* Thursday – Year 4, Year 5 & Year 6
* Friday – KS2 Fitness Club

* DF Next Level Coaching will be running the club.
* The cost per session is £1.00 – (Total cost £7.00 for 7 weeks, to be paid via School Gateway, following confirmation of a place).
* Pupils in Year 1 to Year 6 may attend, please note PLACES ARE LIMITED.
* A message will be sent out via Dojo to confirm if your child has secured a place.

IMPORTANT:
* Please could children bring their own PE Kit from home for the club.
* Children will not be permitted to walk home alone and should be collected from School Gate 1 (Nursery) at 4.10pm promptly.
